TABOR CREST RCF-A MEMORY CARE COMMUNITY

  1. Summary of the care and services we provide.

Tabor Crest RCF-Alzheimer's Endorsed Facility-provides all state required services, for clients diagnosed with Dementia or Alzheimer's Disease, including general assistance with activities of daily living, food service which includes modified special diets as necessary, medication administration assistance, housekeeping services and in facility social and recreational activities.

We assist our clients to schedule outside appointments and assist with scheduling transportation for these appointments.

We are intended to serve independent ambulatory clients, clients that uses assistance devices cane or walker, and clients in wheelchair that can bear weight. We are providing two persons transferring; however, the client must bear weight and not exceed 190Lb.

Tabor Crest RCF provides care for cognitive impaired client with behavior disturbance. The client needs to be manageable by providing redirection, reorientation, engage in distracted activities, one on one for short time periods, manage pain and any trigger discomfort, keep client active with activities of interest, constant interventions, and supervision. The client must respond to interventions and not be a threat to himself or others.

Also, Tabor Crest provides care for clients diagnosed with Diabetes Type 2 besides the Alzheimer's/ Dementia diagnostic, that are on oral medications for their diabetes.

  1. Summary explanation of the types of care and services we do not provide.

Tabor Crest RCF-Alzheimer's Endorsed Facility-does not provide the following services on a routine basis:

    • Medically complex diets beyond the required modified special diets
    • Care for clients that require tight monitoring of their weights-"weight daily" doctor orders
    • Care for clients that their nutrition is through via a tube feeding
    • Care for clients that need medication administration via an IV Line
    • Care for clients that have urostomy or colostomy
    • Care for clients on sliding scale insulin, or difficult to control their CBGs level
    • Care for the clients on Dialysis
    • Care for clients with aggressive behavior that impose a threat to themself and others

Also, our facility does not accept clients with pets or clients that smoke.

We do not provide outings or transport to clients. We do not escort clients to any appointments.

Medications and incontinent supplies are not included in our price; however, we do administrate medications and provide incontinence care.

  1. If your needs exceed the care and services we provide, we may ask you to move out

When your needs exceed the care and services we provide, we will meet with you to discuss the circumstances, attempting to determine the most appropriate care plan and setting to meet your care and service needs. If we cannot properly care for you at our community due to increased needs, we may ask you to move to a more appropriate setting. If an agreement is not reached and attempts to resolve the issue are not successful, we may give you an involuntary move-out notice.

  1. If you leave our community to receive acute medical, psychiatric, nursing facility or other care, we will conduct an evaluation before you return to our community.

Before you return to our community, a qualified staff person will re-evaluate your condition and determine if our community is able to meet your needs. If we determine we can no longer meet your needs, we will issue you an involuntary move out notice, and you will not be permitted to return to our community.

We will notify you of this determination as soon as possible and before you leave the acute care or other setting.

  1. You have the right to ask for an administrative hearing if you disagree with our decision to issue you an involuntary move out notice.

The requirements for requesting a hearing can be found on the Administrative Hearing Request form MSC 0443. You also may contact the Oregon Long Term Care Ombudsman for assistance in requesting a hearing. The phone number for that office is: 800 522 2602 or 503 378 6533.

  1. This is how we arrange for or coordinate hospice care:

Tabor Crest RCF adjust the care and provide care for residents on hospice. Tabor Crest will work with hospice providers to choose one hospice care if you or your representative request it.

When admitting new client in our facility, Tabor Crest will make no discrimination related to national origin, race, color, creed, or gender.
